Sign Up Details

Here is the agenda from the player sign-up meeting today. You need to complete the following three things before you can try out.

1. Sign up using the following link: https://forms.gle/RrZx2xD4PbqtWW9G8 Due ASAP.

2. Complete the following medical clearance and parent permission form. Due November 17

3. Physicals: Passed into the nurse’s office by Friday, Nov 17. Physicals are valid for 13 months.
